Parking
The first two parking spaces on your right are the designated parking spaces for Well. The office is located in suite 100 (walk across the parking lot to the ground-floor entrance).
Downloadable Forms
Download the new patient intake or the pediatric intake form and fax it to (214) 520.9584, so your practitioner may review it before your appointment.
Payment Policy
Payment is due in full at the time of visit. Cash, checks, and major credit cards are accepted.
Cancellation Policy
Patients who do not cancel their appointment within 24 hours will be charged a $50 cancellation fee.
Insurance Coverage
At this time Well does not contract with any insurance company. Please contact your insurance company to find out what services your plan will cover. We will provide you with the necessary bill and coding to file a claim.
